<p>Physical therapy is traditionally practiced in a number of different settings: hospitals, private outpatient clinics and nursing homes, to name a few. The emergence of a new trend in physical therapy now brings physiotherapists directly to the patient’s home to provide treatment. This model entails the physiotherapist traveling to the patient’s home with portable treatment equipment, such as hand held modality equipment.</p>
<p>Studies have shown that the need for home physiotherapy services is increasing. Physiotherapy in the home has been shown to assist clients to gain or re-gain improved strength and mobility, resulting in improved function and a better quality of life. Clients who have had a myocardial infarct and are receiving home care physiotherapy following discharge experience an improved quality of life. Those who have had a stroke benefit from improved mobility, functional capacity, and balance, which in turn reduce the risk of falling.</p>
<p>Although the need for physiotherapy services at home is great, access to quality home and continuing care physiotherapy is limited by the shortage of experienced physiotherapists who provide these services. Patients who received physiotherapy in a hospital setting may not have access to the same services when they are discharged. This can have a significant effect on their ability to achieve optimal function and, in the case of chronic disease, may mean repeated admissions for hospital care.</p>

<p><strong>Benefits to treating patients in their home include:</strong></p>
<p>Many physiotherapists who offer home care treatment have extended hours, allowing for treatment sessions either before or after the patient goes to work. This means that patients don’t need to take time off work in order to receive proper treatment. The physiotherapist evaluates a patient in his or her natural environment. There’s no need for the patient to travel, thus making treatment more time efficient for the patient. Complete privacy for the patient and his or her family. Typically, in home sessions are 1-on-1 with the physiotherapist. This allows for more effective treatment sessions since the physiotherapy performs direct treatment on the client, rather than an assistant. This also gives the patient more time to ask the physiotherapist questions directly.</p>
<p>In-home physiotherapy provides the same quality of treatment that you would receive at a clinic. You don’t have to travel and the home therapist may work around your schedule; In-home physiotherapy will save you time and will be more convenient than going to a clinic.</p>

<p>However, in-home physiotherapy is not for everyone. Before you chose home physiotherapy you should ask yourself several questions:</p>
<p>Are you comfortable with having a stranger coming into your home? Upon the first visit, the physiotherapist will essentially be a stranger coming into your home. The physiotherapist will have experience with home therapy treatments and will try to make the experience as pleasant as possible, but you will be required to divulge some personal information about you and your medical history to the physiotherapist.</p>
<p>Are you capable of concentrating without distractions in your home? Many people have multiple things happening around the home which may lead to distractions during the treatment session. Physiotherapy treatments require your full attention during the treatment session, so you should minimize any possible distractions around your home during the session.</p>
<p>Is my home suitable for home physiotherapy? Physiotherapy can generally be performed in a small area. The physiotherapist will attempt to take advantage of the surrounding environment to administer all the tests and treatment required. Generally, the available space in your home is not an issue; however, your home should have at least a few square feet of open floor space which is accessible to an electrical outlet. The physiotherapist will require an electrical outlet if the use of modalities is required for treatment (i.e. ultrasound). Most of the equipment is light and portable, and will be brought to your home by the physiotherapist.</p>
<p>Is the physiotherapist well trained? Only by utilizing the service from organized home healthcare providers, will ensure quality service to be delivered. The patient should not utilize just anybody but should check with an organization of his or her trust.</p>

With improved control of their blood sugar level and more aggressive management of risk factors for other related complications like macro and microvascular diseases, these patients can benefit in abundance. <p>According to a recent clinic based survey called Screening India’s Twin Epidemic, one in every three adults living adults living in cities suffers from diabetes. 60% of those studied suffered from hypertension or diabetes or both. <p>18.4%, of those tested had a condition called pre-diabetes. Extrapolations from the recent ICMR study say that India has approximately 75 million people suffering from diabetes. But it is estimated that double the number have pre-diabetes. These numbers only show the magnitude of this epidemic facing us.</p>

Mr. Aravind, (name changed), a 45 year old executive working for a MNC was diagnosed as a borderline diabetes in July 2009. However, he did not take the doctor’s advice to keep the disease in control, at that time, due to lack of time and stress related to work. In June 2011, he realized that he has gained 10 kg in the last two years and that he was not feeling fully fit. On consulting the physician again and after performing a battery of tests, he was diagnosed to have Type 2 Diabetes Mellitus, Hypertension and Hypercholesterolemia. This came as a shock to Mr. Aravind and he was not sure how he could manage his lifestyle and take control of his health, given his hectic lifestyle. The physician after prescribing medication for the conditions diagnosed and counselling him, thought it best to refer him to India Home Health Care (IHHC) to seek the partnership of a Registered Nurse to help Mr. Aravind and his wife on how they could tackle the issues at hand and take control. Together with the physician, THE nurse developed a care plan to manage the disease and the comorbidities. The first month, nurse visited them on a daily basis to understand their lifestyle, to educate them on healthy choices, to teach them how to use a glucometer and to record the readings, to ensure medication compliance, to provide constant feedback to the physician on the blood sugar levels in order to titrate the medicine against the levels. By the end of two months, the sugar levels, BP and cholesterol came within the acceptable range. <p><strong>Case Study 2</strong></p>
<p>Mr. Ramakrishna, 80 year old man, living with his 75 year old wife has been a diabetic for the last twenty years. In November 2010, Mr. Ramakrishna met with a road traffic accident and was admitted to a hospital for severe lacerations in his right foot. His fasting blood sugar, at the time of admission, was 200 mg/dl and he was put on insulin to bring down the glucose levels. He had a series of complex surgeries to restore the functions of his foot. However, during the hospital stay, he had developed pressure ulcers and he was apprehensive about using the insulin syringe. Hence, he was discharged to the care of India Home Health Care. IHHC provided him with a 24 hour caretaker to help him with the activities of daily living, a Registered Nurse to visit every day to administer insulin, record his blood glucose levels, dress the pressure ulcers and to watch out for their complications and prevent them and a physiotherapist to visit to get his mobility and strength back. After three months, he was fully mobile and the ulcers were healed. The nurse still visits him every day to administer insulin and check the glucose levels.</p>

Given the attractive Indian consumption story, beauty &amp; wellness businesses are expected to witness immense growth and scale. What are the investible opportunities in this space? What are the challenges engrained this segment and how can they be addressed?</p>
<p>Those where the questions addressed during the panel at the VCCircle Summit on 28.09.2011 in Delhi. The panel headed by Sameer Mehta (Director, Mehta Hospitals) discussed the different challenges the industry is facing. The quest for talent stood at the top of the list. Also the nonexistence of any regulation was addressed, stating that this leaves the unorganized players in the market to operate without ensuring quality results to the patient.</p>

Also setting standards is not possible overnight, in the long run, the organized play will be benefitting. <p>The first National Conference on Ambulatory Surgery and the launch of the Indian Society of Ambulatory Surgery [ISAS] was held in Bangalore on the 2nd and 3rd of September, 2011. The Conference was organized by Nova Medical Centers, pioneers in standalone Ambulatory Surgery Centers in India. The Conference provided a platform for Surgeons across specialties, Physicians, Nurses, Administrators, Quality Assurance Auditors, Biomedical instrument manufacturers and other Healthcare delivery professionals working in Ambulatory Surgery set-ups to share learnings and discuss cases of interest.</p>
<p>Speaking at the conference, Mr. Frank Goller, Chief Executive Officer, India Home Health Care proposed a model that highlighted how home care nursing can benefit ambulatory surgery patients. He mentioned that home care nurses function as eyes and ears of the surgeon and went on to explain how a patient, discharged after a day care surgery, can be assigned a home nurse who will make a visit to the patient and assess the general status of the patient, check the vitals, examine the wound, review the medication, diet compliance and mobilization instructions. Apart from allaying the patient’s anxiety, the visit by the nurse will ensure that post-operative complications are minimized and dealt with immediately. This was received well by the surgeon community.</p>

<p>India Home Health Care, a Chennai-based provider of care services, is expanding services in the South and tying up with corporate sector to offer a range of healthcare assistance.</p>
<p>India Home Health Care provides professional healthcare assistance at home using qualified nurses. It is floated by private promoters with a presence in IT and healthcare.</p>
<p>Tie-ups</p>
<p>Mr Frank Goller, Chief Operation Officer, IHHC, said the company has also tied up with large IT companies and real estate developers to set up first-aid and emergency care services and pre-employment check up in their facilities and large residential complexes that are coming in most major cities. It is present in Chennai and Bangalore and plans to expand its services to Mumbai, Delhi and Hyderabad in the next six months.</p>
<p>India Home Health Care has set up the first of such facilities at the Shriram Gateway IT special economic zone (SEZ), Mr Goller said.</p>
<p>To reach the corporate clients, it has a subsidiary India Corp Healthcare, and is in discussions with the SriCity SEZ to locate a hospital-on-wheels initially. Mr Goller said it collaborates with leading hospitals in Chennai including Fortis Malar, Hande Hospital, Mehtha Hospital, Billroth and Appaswamy Hospitals.</p>
<p>IHHC has a team of over 150-160 qualified nurses and nursing assistants who provide home healthcare, according to Mr V. Thyagarajan, Director, IHHC, and a promoter. Zymbii Home Healthcare, a US-based service provider, is in technical collaboration for processes and services, he said. IHHC&#8217;s staff will work closely with the treating doctor to support the patients, he said.</p>
