Palliative Care At Home
Palliative hospice care offers services to help relieve suffering or soothing of symptoms of disease or a disorder while maintaining the highest possible quality of life for patients. This approach becomes appropriate when treatment no longer supports quality of life. This is to move away from getting better to palliative or comfort care. This is a specific approach to care for people who have a life limiting illness or are terminally ill.
Palliative care can be best delivered if there is a team approach who can prescribe the treatment, assist with medical decisions, coordinate with the needy person with other Doctors to help navigate, making a plan for living will and give emotional/spiritual support.

Palliative care can be given at home, community, assisted care living facilities, residential care facilities and in hospice residencies.
Home is the place where the heart is. If the medical team can come home and give palliative care at home then that will be the best care one can give for a ailing person. For example a 91 year old obese person is bedridden with cellulitis in her both lower extremities and has 4th degree pressure sore in the sacral region. She has a team of a Doctor ( experienced Geriatrician), a Registered Nurse,2 Associate Nurse and a Medical care taker to take care of her. They do give personal hygiene, wound debridement and dressing on a regular basis, transfers to the wheelchair to get some fresh air and look at the garden and give food and medicines on time. The family is giving an excellent palliative care at home for this particular person.
