Our standard process for initiating service is as follow:
The First Call
Upon receiving the first call for service, IHHC’s clinically trained Operations team gathers initial information on the patient and his/her requirement. The staff will then determine which care provider will best suit the patient’s needs and fix up an appointment for our senior staff and the assigned care provider to make the initial visit.
Initial Assessment
A senior member of the operations team and the assigned care provider visit the patient’s house; do a thorough initial assessment of the patient and the home environment using internationally accepted best practices.
Care Plan
Upon performing the initial assessment, the team draws up a personalized care plan for the patient, in conjunction with the treating physician, depending on his/her availability. The Care Plan will detail out the specific services that will be provided. We will also be happy to involve the immediate family members in the Care Planning.
Care Follow Up
We monitor the services provided on a consistent basis through home visits by our Quality Assurance Team and through phone calls with the patient and staff at the patient’s bedside. Any quality related issue will be addressed within 24 hours.
